New RFID Tag Request for Gate Entry

This form is for Lake Sheen Reserve residents to request new RFID tags for newly registered vehicles. Each RFID tag is issued at a cost of $15, payable only by check made out to “Lake Sheen Reserve HOA.” Once all required vehicle information and payment are received, processing and issuance may take up to 14 days.
